At Healthy Communities, we understand that building better communities starts with building better lives. Better lives through better physical and mental health, better lives through stronger families, better lives through safer neighborhoods.

■ Better health starts with quality care with providers in a primary care medical home, where you’re not just another patient ID number. Healthy Communities’ highly-skilled staff serves a diverse patient base with needs that range from simple to complex.

■ Better lives start with building stronger families, and through the Faith Advisory Council’s Faith Initiative we work with congregations across Alameda County to recruit foster and adoptive families to provide safe and loving homes for the almost 1,500 children currently in foster care.

■ Safer neighborhoods support building better lives and our Measure Y-funded violence prevention street outreach teams fan out across the hotspots in East and West Oakland every week to help bring peace back to our neighborhoods.

■ At Healthy Communities we offer services to address a wide range of concerns and needs for our clients, whether under-insured or uninsured, homeless, low-income, reentry, urban males, and many others. It’s about treating the whole person.
